PERSONAL

Original copy of Electrical Permit with endorsement from OBO (Office of the Building Officials)
Load Listing
Photocopy of 1 Valid Government-Issued ID
Proof of ownership (either of the following):
- Condominium Certificate of Title
- Contract to Sell and Certification from the Developer or Authority to move in
- Deed of Sale
If applicant is not available to sign the Electric Service Agreement and Data Privacy Form )
- Notarized Special Power of Attorney
- Valid Government-Issued ID of authorized representative
Pay the required Bill Deposit
Settle any outstanding balance of the existing account
